When the record and the event stop matching
The gap between having a record and having a record that tells the truth shows up well outside maritime too, and a recent example makes the pattern easy to see. Reporting on Meta's own documentation for its AI system Muse noted that having a record isn't the same as the record telling the truth: of two published documents covering the same product, only one actually referenced the security incidents that had occurred. Both documents existed. Only one of them matched reality.
An SMS runs on the same logic, at a smaller scale, many times a week. A drill log, a PMS closure, a near-miss review: each one either matches what happened on board or it doesn't, and a sign-off alone cannot tell an auditor which is the case.
Why this matters more in an SMS than almost anywhere else
The ISM Code builds an entire safety case on a chain of sign-offs: drills conducted and debriefed, planned maintenance completed and verified, near misses reviewed and closed with a root cause identified, corrective actions checked and confirmed. Every one of those steps produces a record, and the SMS treats the record as the evidence that the underlying activity took place.
Under real operational pressure, that chain is exactly where drift creeps in. A drill gets logged against the schedule because the crew ran out of time to run it properly this rotation. A maintenance task gets marked closed to keep the backlog number down, with the actual work queued for later. A near-miss gets closed out to hit a monthly target before the investigation has actually established a cause. None of these are dramatic falsifications. They are small, familiar compromises that leave the SMS looking exactly as compliant as it would if none of them had happened.
Where sign-off drift shows up in a typical SMS

None of these rows require bad faith to happen. Time pressure, understaffed rotations and KPI targets built around closure rates rather than accuracy will produce this pattern on their own, in any fleet, without anyone deciding to falsify anything.
Where SMS implementation itself needs to catch this
This is a design problem as much as a behavioural one, and it is easiest to fix at the point an SMS is actually built or rebuilt rather than after drift has already set in. Getting sign-off integrity right at that stage means building in verification that does not rely purely on trust: timestamps that cannot be backdated, evidence attached to the sign-off rather than assumed alongside it, and workflows that make it harder to close a task than to actually complete it. An SMS bolted together around sign-off boxes with nothing behind them will always be vulnerable to this drift, no matter how well the underlying safety procedures are written.
What actually closes the gap
Closing the gap between the record and the event means treating the sign-off as evidence to be checked, not evidence in itself. Audits that sample against operational reality, spot-checking whether a logged drill actually has a debrief behind it or a closed maintenance task actually has a completed job card, catch drift that a purely document-based audit never will. So does designing the sign-off step itself to require more than a signature: a photo, a linked inspection record, a timestamp that ties back to when the work was genuinely done.
A fully signed-off SMS and a genuinely safe vessel are supposed to be the same thing. They only stay the same thing if every sign-off in between is still telling the truth.
