Module description
The CMS Patient Safety Structural Measure is not a documentation exercise. CMS is looking for evidence that patient safety is structurally embedded in how your organization operates: that leadership is accountable, that staff feel safe to report, that data is used to drive learning, and that improvement is continuous rather than reactive.
The PSSM measurement period is the calendar year, and hospitals attest each spring through the CDC National Healthcare Safety Network (NHSN). Hospitals must self-attest that they meet the structural requirements across all five domains. Beginning in fall 2026, each hospital's score is publicly reported on Care Compare on a 0 to 5 scale, one point per domain, and scoring within each domain is all or nothing.
